Dear Sir,
The Old Comrades Association
I am desirous of becoming a member of the above Association
and I hereby authorise the deduction of 30p per month, or the rate
fixed by the membership at the Annual General Meeting of the Association,
be deducted from each monthly amount of my pension, in payment
of membership subscriptions. The subscription to be paid to the Treasurer
of the Association.
